As a valuable person in the Organization / Department you are invited to join other employees in a very important project. The honest responses to the questions in this survey will provide important information that will be utilized in the development of a series of action plans for change management and/or process improvement initiatives within your organization.
The purpose of the survey is to identify current perceptions and attitudes of employees about the organization’s culture, values, work structures, communications and readiness for change.
Please respond honestly to the questions. The responses you provide will be compiled with those of other employees.
Organizational Effectiveness Survey Instructions
- We encourage you to answer all the questions which pertain to your experience within the Organization / Department for the last 60 to 90 days. We want your perceptions about the Organization / Department and not what you think your boss or co-workers would want you to say. The best way to do this is to mark down the first response that comes into your mind. If you cannot decide on an answer, go on to the next one but be careful that the next space you mark has the same number as the question you are answering.
- Please read the directions carefully. Links to the Definition of Terms are located on the upper portion of each page, represented by the
icon.

- Most people will complete the survey in 10-15 minutes. However, the time limit for taking the survey is 1 hour. The web server will time-out your session after 60 minutes. Please start over at the beginning should an interruption keep you from completing within 60 minutes.
- Please carefully answer before moving to the next page. Once moved to the next page, answers are saved. If you need to go back to a previous page, use your browser's back button. If changes are made to the page returned to, re-submit these changes by using the next page button at the bottom of the survey. Moving to the next page will cause a refresh of that page.
