This Master Service Agreement (the "MSA") sets out the terms by which Performance Dashboard, an Oklahoma Limited Liability Company ("PD") provides all services and products to the Client, as identified by the Client in any PD authorized Client Request for Services Form or similar form found at https://www.performancedashboard.com/secure/reports.asp and submitted by the Client to, and accepted by PD whether one or more, or by request by the Client and acceptance by PD (collectively, the "Client Request") which is made a part of, and incorporated into this MSA.
