The purpose of this employee feedback inventory is to help you determine the level of work performance of a person who works for you. Your responses to the questions in this survey will provide your assessment of this employee’s performance during the past 12 months. The employee will also be providing his or her self-rating on these same questions. You will receive a report that compares your assessment to the employee’s self-rating. This report will provide you with critical information that can be used as a basis for employee performance discussions and supervisor expectations for future performance. In addition, the data gathered from this report will be used to help identify employees who may be considered for a bonus award at the end of the fiscal year.
Before completing this survey recall the employee’s performance within the last 12 months.
Instructions
- We encourage you to answer all the questions, which pertain to your experience about the performance of the person who works for you in last 12 months. We want your perceptions and not what you think your supervisor or peers would want you to say. The best way to do this is to mark down the first response that comes into your mind.
- Please read the directions carefully. Links to the Definition of Terms are located on the upper portion of each page, represented by the
icon. Links to the Survey Instructions are also located on the upper portion of each page, represented by the icon.
- Most people will complete the Feedback Performance Review survey in 10-15 minutes. However, the time limit for taking the survey is 1 hour. The web server will time-out your session after 60 minutes. Please start over at the beginning should an interruption keep you from completing within 60 minutes.
- Please carefully answer before moving to the next page. Once moved to the next page, answers are saved. If you need to go back to a previous page, use your browser's back button. If changes are made to the page returned to, re-submit these changes by using the next page button at the bottom of the survey. Moving to the next page will cause a refresh of that page.
