The purpose of this employee feedback inventory is to help you determine your own level of performance during the past 12 months, as well as your perception of the performance of your supervisor and the senior leadership of your company. Your responses to the questions in this survey will help you, your supervisor, and senior leadership to receive feedback on important work related areas that your company has determined are important to effective performance.

Instructions

  • Please answer all of the questions that pertain to your own performance. Try to answer all of the questions about your supervisor and senior leadership. If you are unable to answer a question, click on the "Don’t Know" bubble. We want your perceptions and not what you think your supervisor or co-workers would want you to say. The best way to do this is to answer according to the first response that comes into your mind.
  • Please read the directions carefully. Links to the Definition of Terms are located on the upper portion of each page, represented by the Definition of Terms icon. Links to the Survey Instructions are also located on the upper portion of each page, represented by the Survey Instructions icon.
  • Most people will complete the eFeedback Performance Review survey in 10-15 minutes. However, the time limit for taking the survey is 1 hour. The web server will time-out your session after 60 minutes. Please start over at the beginning should an interruption keep you from completing within 60 minutes.
  • Please respond to all questions on each part before clicking on the Next Page icon. Once moved to the next page, answers are saved. If you need to go back to a previous page, use your browser's back button. If changes are made to the page returned to, re-submit these changes by using the next page button at the bottom of the survey. Moving to the next page will cause a refresh of that page.
